Below you will find the Adult Medical & Dental History form and the Orthodontic Examination Questionnaire. Please feel free to download these forms, print and fill them out prior to your first visit. 1. What if the bands, brackets or expander becomes loose?

Leave the band/bracket on the wire. Call our office to schedule an appointment to have Dr. Chubb reconnect it.

2. What if the archwire, hook or elastic tie are broken or lost?

This could cause the teeth to shift to one side. Call our office to schedule an appointment to replace or repair the problem.

3. What if there is a wire or hook sticking out and poking?

Try to tuck it in with the eraser end of a pencil. If that doesn't work, try to place wax over the area sticking out.

4. What if my teeth feel sore?

Take Advil™ or Tylenol™ as prescribed. You may also rinse with warm salt water to relieve any sore areas on the inside lips or cheeks.

5. What are the names of the different parts of the braces?

A.	Bracket - The attachment bonded to the teeth or welded to the band
B.	Archwire - A large removable wire that fits around the arch into the bracket slots
C.	Elastic Tie - Plastic ring that ties archwire into bracket or Ligature Wire - Tiny wire that ties archwire into bracket
D.	Hooks - Used to attach elastics (rubber bands)
E.	Band - A ring of metal with the bracket attached that is glued on the tooth
